FISCAL IMPLICATIONS

Funds for the SWA are already available in Capital Project 263-CP19003 Relocate 2nd Floor Dental Suite at MJN.

CONTRACT HISTORY

On November 20, 2018, Item 23, The Board of Supervisors awarded the contract in the amount of $809,000 to the lowest responsive bidder, Agbayani Construction Corporation, with a construction time of 160 working days.

REASONS FOR RECOMMENDATION

During construction, unforeseen conditions arose with the routing of the utility lines and access flow for the final selected dental equipment which resulted in additional design and construction modifications. The previous approved SWA has been exhausted to support other unforeseen conditions and operational impacts to the construction schedule. Thus, the SWA needs to be increased to support the project.

BACKGROUND

The Relocation of the suite will replace a single exam station with a 3-Dimensional imaging station, two Americans with Disabilities Act (ADA) compliant examination stations, and a denture lab.

CONSEQUENCES OF NEGATIVE ACTION

The increase to the SWA would not be approved at this time.
